Zantac 360 1 Per Blister 6 Blisters

The dosage of famotidine depends entirely on the condition being treated and the severity of the symptoms. Healthcare providers typically follow these standard guidelines for adults:

• Active Duodenal Ulcer: The standard dose is 40 mg once daily at bedtime or 20 mg twice daily. Most ulcers heal within 4 weeks, though some may require up to 8 weeks.
• Maintenance of Healed Duodenal Ulcer: To prevent the ulcer from returning, a reduced dose of 20 mg once daily at bedtime is common.
• Active Benign Gastric Ulcer: The typical dose is 40 mg once daily at bedtime.
• Gastroesophageal Reflux Disease (GERD): For the treatment of symptoms, 20 mg twice daily for up to 6 weeks is common. For the treatment of erosive esophagitis, the dose may be increased to 20 mg or 40 mg twice daily for up to 12 weeks.
• Pathological Hypersecretory Conditions (e.g., Zollinger-Ellison): Dosing is highly individualized. It often starts at 20 mg every 6 hours, but doses up to 160 mg every 6 hours have been used in severe cases.
• OTC Heartburn Prevention: 10 mg to 20 mg taken 15 to 60 minutes before eating food or drinking beverages that cause heartburn.

Famotidine is approved for use in pediatric patients ranging from neonates to adolescents, though the dosing is strictly weight-based and must be managed by a pediatrician.

• Peptic Ulcer (Ages 1-16): 0.5 mg/kg/day at bedtime or divided into two doses, up to a maximum of 40 mg per day.
• GERD (Ages 1-16): 1 mg/kg/day divided into two doses, up to a maximum of 40 mg twice daily.
• Infants (under 1 year): Use is specialized for GERD, typically 0.5 mg/kg per dose. Parents must follow the specific liquid formulation instructions provided by the pharmacist.

Renal Impairment

Because famotidine is primarily cleared by the kidneys, patients with moderate to severe renal impairment (creatinine clearance < 50 mL/min) require significant dosage adjustments. In these patients, the dose may be reduced by 50%, or the dosing interval may be extended to every 36 to 48 hours. Failure to adjust the dose can lead to accumulation of the drug and increased risk of central nervous system side effects like confusion or hallucinations.

Hepatic Impairment

Generally, no dosage adjustment is required for patients with mild to moderate liver disease, as the liver plays a secondary role in famotidine's elimination. However, caution is always advised in patients with severe hepatic failure.

Elderly Patients

Older adults are more likely to have decreased renal function. Therefore, healthcare providers often start at the lower end of the dosing range and monitor kidney function closely. The risk of CNS side effects (confusion, agitation) is significantly higher in this population.

To achieve the best results, patients should follow these administration guidelines:

• Consistency: Take the medication at the same time(s) each day. If taking once daily, the bedtime dose is often most effective for controlling nighttime acid.
• With or Without Food: Famotidine can be taken regardless of meals. However, if using it to prevent heartburn from specific foods, take it 15-60 minutes before the meal.
• Oral Suspension: Shake the bottle well for 5-10 seconds before each use. Use a calibrated measuring device (not a kitchen spoon) to ensure the correct dose.
• Tablets: Swallow tablets whole with a glass of water. Do not crush or chew unless specifically instructed by your doctor (though some OTC forms are chewable).
• Storage: Store at room temperature (68°F to 77°F) away from moisture, heat, and light. Keep the liquid suspension from freezing.

If you miss a dose, take it as soon as you remember. However, if it is almost time for your next scheduled dose, skip the missed dose and return to your regular schedule. Do not 'double up' or take two doses at once to compensate for a missed one.

While famotidine has a wide safety margin, an overdose can occur. Symptoms of overdose may include extreme dizziness, fainting, or unusual heart rate changes. In the event of a suspected overdose, contact a poison control center (1-800-222-1222) or seek emergency medical attention immediately.

> Important: Follow your healthcare provider's dosing instructions. Do not adjust your dose without medical guidance. Long-term use of acid reducers should be supervised by a professional to ensure underlying conditions are not being masked.

Famotidine is generally well-tolerated by the majority of patients. However, some individuals may experience mild side effects as their body adjusts to the medication. Common side effects include:

• Headache: This is the most frequently reported side effect. It is usually mild and transient.
• Dizziness: Some patients may feel lightheaded, especially when moving from a sitting to a standing position.
• Constipation or Diarrhea: Changes in bowel habits can occur due to the alteration of the stomach's pH balance.
• Dry Mouth: A sensation of dryness in the oral cavity is occasionally noted.

These symptoms often resolve on their own within a few days. If they persist or become bothersome, consult your healthcare provider.

• Fatigue and Asthenia: A general feeling of weakness or lack of energy.
• Muscle Cramps: Occasional aches or spasms in the legs or arms.
• Nausea and Vomiting: While famotidine treats GI issues, it can sometimes cause mild stomach upset in sensitive individuals.
• Anorexia: A slight decrease in appetite.

• Central Nervous System Effects: These are more common in the elderly or those with kidney disease and include confusion, hallucinations, agitation, depression, and anxiety.
• Cardiac Arrhythmias: Very rare reports of prolonged QT interval, especially in patients with pre-existing heart conditions or renal failure.
• Hematologic Changes: Rare instances of agranulocytosis, pancytopenia, or thrombocytopenia (low blood cell counts).
• Hepatic Issues: Rare elevations in liver enzymes or cholestatic jaundice (yellowing of the skin/eyes).

While rare, some side effects require urgent medical intervention.

> Warning: Stop taking Famotidine and call your doctor immediately if you experience any of the following:

1. 1Hypersensitivity Reactions: Signs include hives, skin rash, swelling of the face, lips, tongue, or throat, and difficulty breathing or swallowing. This may indicate anaphylaxis.
2. 2Severe Skin Reactions: Stevens-Johnson Syndrome (SJS) or Toxic Epidermal Necrolysis (TEN) have been reported. Look for blistering, peeling skin, or red/purple rashes.
3. 3Neurological Changes: Sudden confusion, disorientation, hallucinations, or seizures.
4. 4Cardiac Symptoms: Fast, pounding, or irregular heartbeats, or sudden fainting spells.
5. 5Signs of Infection: Unexplained fever, sore throat, or extreme chills (which may indicate low white blood cell counts).
6. 6Easy Bruising or Bleeding: Unusual bleeding from the gums or nose.

When used for extended periods (typically more than 2 years), famotidine and other acid-suppressing drugs can lead to specific complications:

• Vitamin B12 Deficiency: Stomach acid is required for the absorption of Vitamin B12 from food. Chronic acid suppression can lead to a deficiency over time, resulting in anemia or nerve damage.
• Magnesium Deficiency (Hypomagnesemia): Although more common with PPIs, long-term H2RA use has been associated with low magnesium levels, which can cause muscle tremors and heart palpitations.
• Increased Risk of Infection: Stomach acid serves as a barrier against bacteria. Reducing this acid may slightly increase the risk of developing community-acquired pneumonia or gastrointestinal infections like Clostridioides difficile (C. diff).
• Bone Fractures: Some observational studies suggest a potential link between long-term acid suppression and an increased risk of hip, wrist, or spine fractures, likely due to interference with calcium absorption.

There are currently no FDA black box warnings for famotidine. It is considered one of the safer profile medications in the H2RA class. However, the absence of a black box warning does not mean the drug is without risk; it simply means the risks do not meet the specific threshold for the FDA's most stringent warning category.

Report any unusual symptoms to your healthcare provider. Keeping a symptom diary can help your doctor determine if a side effect is related to the medication or another underlying condition.

Before starting famotidine, it is crucial to understand that while it is effective for symptomatic relief, it may mask the symptoms of more serious underlying conditions. For instance, the relief of symptoms does not preclude the presence of gastric malignancy (stomach cancer). If symptoms persist despite treatment, or if you experience 'alarm symptoms' like unexplained weight loss, persistent vomiting, or difficulty swallowing, further diagnostic testing (such as an endoscopy) is necessary.

No FDA black box warnings for Famotidine. This medication has maintained a strong safety record since its introduction in the 1980s.

• Allergic Reactions: Patients with a known hypersensitivity to famotidine or other H2 blockers (such as ranitidine or cimetidine) should avoid this drug. Cross-sensitivity among this class of drugs has been observed.
• Renal Dysfunction: This is the most significant precaution. Because the kidneys are the primary route of elimination, famotidine can reach toxic levels in patients with impaired kidney function. This accumulation is directly linked to CNS toxicity, including confusion and delirium.
• CNS Effects: Even in patients without renal issues, famotidine can occasionally cause CNS depression. Caution is advised when performing tasks that require mental alertness.
• QT Prolongation: There have been rare reports of QT interval prolongation in patients with renal impairment who were given doses higher than recommended. This can lead to a dangerous heart rhythm known as Torsades de Pointes.

For short-term OTC use, regular lab monitoring is generally not required. However, for patients on long-term prescription therapy or those with underlying health issues, healthcare providers may monitor:

• Renal Function Tests: Periodic checks of Serum Creatinine and Glomerular Filtration Rate (GFR) to ensure the dosage remains appropriate.
• Complete Blood Count (CBC): To monitor for rare blood dyscrasias (abnormalities).
• Liver Function Tests (LFTs): To ensure the liver is processing the medication correctly.
• Vitamin B12 Levels: For patients on therapy exceeding one to two years.

Famotidine may cause dizziness or drowsiness in some individuals. Until you know how this medication affects you, use caution when driving, operating heavy machinery, or engaging in other hazardous activities. If you experience confusion or blurred vision, stop these activities immediately and contact your doctor.

While there is no direct chemical interaction between famotidine and alcohol, alcohol is a known gastric irritant. Consuming alcohol can worsen the symptoms of GERD and ulcers, effectively working against the therapeutic goals of famotidine. Furthermore, both alcohol and famotidine can cause dizziness; combining them may increase this effect.

For most patients, famotidine can be stopped without a complex tapering schedule. However, some individuals may experience 'rebound acid hypersecretion'—a temporary increase in acid production after stopping the drug. If you have been taking high doses for a long time, your doctor may suggest gradually reducing the dose to prevent a sudden return of symptoms.

> Important: Discuss all your medical conditions with your healthcare provider before starting Famotidine. Be sure to mention if you have kidney disease, liver disease, or a history of long QT syndrome.

There are few absolute contraindications for famotidine, but certain combinations are highly discouraged:

• Pazopanib: Famotidine can significantly decrease the absorption of this cancer medication, potentially reducing its effectiveness. They should generally not be used together.
• Delavirdine: The absorption of this antiviral depends on an acidic stomach environment. Famotidine can lead to subtherapeutic levels of delavirdine, risking viral resistance.

• Atazanavir and Ritonavir: These HIV medications require stomach acid for absorption. Taking famotidine can lower their blood levels, potentially leading to treatment failure. If they must be used together, specific timing (e.g., taking the H2RA 10 hours after the antiviral) is required.
• Dasatinib, Erlotinib, and Gefitinib: These tyrosine kinase inhibitors (used in cancer) have pH-dependent solubility. Famotidine can significantly reduce their bioavailability.
• Tizanidine: Famotidine may increase the plasma concentration of tizanidine (a muscle relaxant), leading to excessive sedation and low blood pressure.

• Ketoconazole and Itraconazole: These antifungal drugs need acid to dissolve. Famotidine can reduce their absorption by up to 80%. It is often recommended to take these antifungals with an acidic beverage (like cola) if famotidine cannot be discontinued.
• Iron Salts (Ferrous Sulfate): Iron absorption is optimized in an acidic environment. Famotidine may reduce the efficacy of iron supplements.
• Cefditoren and Cefpodoxime: The absorption of these antibiotics may be reduced by famotidine.

• Caffeine: Famotidine does not interact chemically with caffeine, but caffeine stimulates acid production and can worsen the symptoms famotidine is trying to treat.
• High-Fat Meals: While food does not significantly affect famotidine absorption, heavy, fatty meals can trigger reflux symptoms.
• Dairy: There is no specific interaction with dairy products, unlike some antibiotics.

• St. John's Wort: While famotidine is not a major CYP3A4 substrate, St. John's Wort can affect overall gastric motility and should be discussed with a doctor.
• Calcium Carbonate: Often found in supplements and antacids. While it can be taken with famotidine, taking them at the exact same time might slightly alter the absorption rate of the tablet.

• Gastric Acid Secretion Tests: Famotidine will interfere with the results of tests designed to measure how much acid the stomach produces. It should be discontinued 24 hours prior to such tests.
• Skin Antigen Tests: Some H2 blockers can slightly suppress the 'wheal and flare' reaction in allergy skin tests. While this effect is much stronger with H1 blockers (like Benadryl), it is best to inform your allergist if you are taking famotidine.
• Urine Protein Tests: Rare reports suggest H2 blockers might cause false-positive results in some tests for protein in the urine.

For each major interaction, the primary mechanism is usually the alteration of gastric pH. By raising the pH (making the stomach less acidic), famotidine changes the solubility and absorption kinetics of other drugs. Unlike cimetidine, famotidine does not significantly inhibit the cytochrome P450 enzymes, which is why it has fewer metabolic interactions.

> Important: Tell your doctor about ALL medications, supplements, and herbal products you are taking. This includes vitamins and over-the-counter pain relievers.

Famotidine must NEVER be used in the following circumstances:

• Hypersensitivity to Famotidine: Any patient who has had a severe allergic reaction (anaphylaxis, angioedema) to famotidine must not take it again.
• Cross-Sensitivity: Patients who have had a serious reaction to other H2-receptor antagonists (such as cimetidine, nizatidine, or the now-recalled ranitidine) should not take famotidine. The molecular structures are similar enough that a cross-allergic reaction is a significant risk.

These are conditions where the risk may outweigh the benefit, or where extreme caution is required:

• Moderate to Severe Renal Impairment: While not an absolute contraindication, it is a 'red flag' condition. Without careful dose reduction, the risk of CNS toxicity is unacceptably high.
• Gastric Malignancy: If a patient has a known or suspected stomach tumor, famotidine should be used with caution as it can improve symptoms and delay the diagnosis of cancer.
• Elderly Patients with Dementia: Because famotidine can cause confusion, it may worsen the cognitive status of patients already suffering from dementia or other neurodegenerative diseases.
• Long QT Syndrome: Patients with a history of heart rhythm problems or those taking other medications that prolong the QT interval should be evaluated carefully before using high-dose famotidine.

As mentioned, the H2RA class shares a similar chemical 'backbone.' If you have experienced a skin rash, hives, or breathing trouble with any 'tidine' drug, you must inform your doctor. This does not necessarily apply to 'prazole' drugs (PPIs), as they belong to a different chemical class, but a full allergy history is essential for safe prescribing.

> Important: Your healthcare provider will evaluate your complete medical history before prescribing Famotidine. Never start this medication if you have a history of severe allergies to acid reducers.

Famotidine was traditionally classified as FDA Pregnancy Category B. This means that animal reproduction studies have failed to demonstrate a risk to the fetus, but there are no adequate and well-controlled studies in pregnant women.

• First Trimester: Most clinicians consider famotidine safe for use when lifestyle changes (like smaller meals) fail to control severe reflux.
• Teratogenicity: Current data does not suggest an increased risk of major birth defects or miscarriage associated with famotidine use.
• Clinical Use: It is often preferred over PPIs in pregnancy due to a longer history of safety data, though both are generally considered low-risk.

Famotidine is excreted in human breast milk. However, the amounts are very small and are unlikely to cause adverse effects in a nursing infant. The American Academy of Pediatrics considers famotidine to be compatible with breastfeeding. Nevertheless, mothers should monitor their infants for any changes in feeding habits or unusual irritability.

Famotidine is approved for use in children from birth to 16 years of age.

• Infants: It is frequently used for 'spitting up' or GERD that causes poor weight gain or respiratory issues.
• Safety: The safety profile in children is similar to that in adults. Long-term effects on bone development or the gut microbiome in children are still being studied, so the 'lowest effective dose for the shortest duration' rule is strictly followed.

In the elderly (ages 65 and older), famotidine must be handled with care:

• Renal Clearance: Natural age-related decline in kidney function increases the risk of drug accumulation.
• CNS Sensitivity: The elderly are much more susceptible to famotidine-induced confusion, delirium, and agitation. This is often mistaken for dementia or 'sundowning.'
• Fall Risk: Dizziness caused by the drug can increase the risk of falls and subsequent fractures.

As the primary elimination organ, the kidneys' health is paramount. For patients with a GFR < 50 mL/min, the dose should be reduced by half or the interval doubled. In patients on hemodialysis, famotidine is not significantly removed by the dialysis process; therefore, the adjusted dose should be given after the dialysis session.

For patients with liver cirrhosis or other hepatic issues, famotidine is generally safe. Unlike cimetidine, it does not require extensive liver metabolism. No specific dose adjustments are typically required for Child-Pugh Class A or B patients, though Class C (severe) patients should be monitored closely.

> Important: Special populations require individualized medical assessment. Always consult a specialist when treating infants, the elderly, or pregnant individuals.

Famotidine is a competitive, reversible inhibitor of histamine at the H2 receptors. These receptors are G-protein coupled receptors located on the parietal cells of the stomach. Under normal conditions, histamine is released from enterochromaffin-like (ECL) cells and binds to the H2 receptor, activating the Gs protein. This stimulates adenylate cyclase, increasing intracellular cAMP. The rise in cAMP activates protein kinase A, which then triggers the H+/K+ ATPase pump (the proton pump) to move hydrogen ions into the stomach. By blocking the H2 receptor, famotidine prevents this entire signaling cascade. It is highly selective and does not affect H1 receptors (involved in allergies) or H3/H4 receptors.

• Onset of Action: Oral famotidine begins to reduce acid secretion within 1 hour of administration.
• Peak Effect: The maximum acid suppression occurs between 1 and 3 hours.
• Duration: A single 20 mg or 40 mg dose can suppress acid for 10 to 12 hours. This makes it ideal for 'nocturnal acid breakthrough.'
• Tolerance: Some patients may develop 'tachyphylaxis' (a rapidly diminishing response to the drug) after 1-2 weeks of continuous use. This is why it is often used intermittently rather than continuously for years.

• Molecular Formula: C8H15N7O2S3
• Molecular Weight: 337.45 g/mol
• Solubility: Famotidine is freely soluble in glacial acetic acid, slightly soluble in methanol, and very slightly soluble in water.
• Structure: It contains a guanidinothiazole ring and a sulfamoyl group, which contributes to its high potency compared to other H2RAs.

Famotidine is classified as a Histamine-2 Receptor Antagonist (H2RA). It is part of the 'tidine' family, which includes cimetidine, nizatidine, and the discontinued ranitidine. Within this class, famotidine is the most potent on a milligram-for-milligram basis, being approximately 20 to 50 times more potent than cimetidine.
