Urelle

Dosage must be individualized based on the severity of symptoms and the patient's tolerance to side effects. According to clinical guidelines, the following are standard ranges:

• Immediate-Release (Tablets, Sublingual, Elixir): The typical adult dose is 0.125 mg to 0.25 mg (1 to 2 tablets) every four hours or as needed. The maximum daily dose should not exceed 1.5 mg (12 tablets) in a 24-hour period.
• Extended-Release (Sustained-Release): The standard dose is 0.375 mg taken every 12 hours. In some cases, your doctor may increase this to 0.75 mg every 12 hours, but the total daily dose should not exceed 1.5 mg.
• Peptic Ulcer Disease: Often used as an adjunct to antacids or H2 blockers, with dosing typically occurring 30 to 60 minutes before meals.

Pediatric use requires extreme caution due to the risk of anticholinergic toxicity. Dosing is strictly based on body weight.

• Infants and Children: For infant colic, the dose is often 0.005 mg/kg per dose. For children weighing 10 kg, the dose is approximately 0.05 mg.
• Caution: Many healthcare providers avoid the use of hyoscyamine in infants under 6 months of age due to reports of respiratory distress and seizures. Always use the calibrated dropper provided with the liquid medication to ensure accuracy.

Renal Impairment

Hyoscyamine is primarily excreted by the kidneys. In patients with significant renal impairment (decreased GFR), the drug may accumulate, leading to increased side effects. Lower starting doses and extended dosing intervals are generally recommended. Clinical monitoring for signs of toxicity (confusion, urinary retention) is essential.

Hepatic Impairment

While the liver plays a role in metabolism, renal excretion is the dominant pathway. However, in patients with severe liver disease, drug clearance may be slightly reduced. Dose adjustments should be made cautiously based on clinical response.

Elderly Patients

Patients over the age of 65 are significantly more sensitive to the effects of hyoscyamine. The Beers Criteria for Potentially Inappropriate Medication Use in Older Adults lists hyoscyamine as a drug to avoid if possible, due to the high risk of confusion, falls, and urinary retention. If used, the lowest possible dose should be initiated.

• Timing: For gastrointestinal spasms, take the medication 30 to 60 minutes before meals for optimal efficacy in preventing post-meal cramping.
• Sublingual Use: Place the sublingual tablet under the tongue and allow it to dissolve completely. Do not swallow the tablet whole if it is designed for sublingual use.
• Extended-Release: Swallow these tablets whole. Do not crush, chew, or break them, as this will release the entire dose at once, increasing the risk of severe side effects.
• Storage: Store at room temperature (20°C to 25°C / 68°F to 77°F) in a dry place away from direct sunlight. Keep the bottle tightly closed.

If you miss a dose, take it as soon as you remember. If it is almost time for your next scheduled dose, skip the missed dose and return to your regular schedule. Never double the dose to 'catch up,' as this significantly increases the risk of an anticholinergic overdose.

Hyoscyamine overdose can be life-threatening and presents as an 'anticholinergic crisis.' Symptoms include:

• Severe dilated pupils (mydriasis) and blurred vision
• Extremely dry skin and mouth
• Rapid heart rate (tachycardia)
• High fever (hyperpyrexia)
• Confusion, hallucinations, or seizures
• Urinary retention and paralytic ileus (bowel paralysis)

In the event of a suspected overdose, contact your local poison control center or seek emergency medical attention immediately. Treatment often involves supportive care and, in severe cases, the administration of physostigmine, a reversible cholinesterase inhibitor.

> Important: Follow your healthcare provider's dosing instructions. Do not adjust your dose without medical guidance.

Because hyoscyamine blocks acetylcholine throughout the body, side effects are common and predictable based on its mechanism of action. These include:

• Dry Mouth (Xerostomia): This is the most frequent side effect. It can make swallowing or speaking difficult and may increase the risk of dental cavities over time.
• Blurred Vision: Due to the relaxation of the ciliary muscle in the eye (cycloplegia) and dilation of the pupil (mydriasis). This can make reading or driving difficult.
• Constipation: Reduced intestinal motility leads to harder stools and infrequent bowel movements.
• Drowsiness: Hyoscyamine can cross into the brain, causing a sedative effect in many patients.
• Decreased Sweating: This can lead to heat intolerance and an increased risk of heatstroke in warm environments.

• Urinary Hesitancy: Difficulty starting urination or a weak stream, particularly in men with an enlarged prostate.
• Tachycardia: A noticeable increase in heart rate or palpitations.
• Headache: Often related to changes in ocular pressure or systemic effects.
• Nervousness or Insomnia: Some patients experience paradoxical excitation rather than drowsiness.
• Taste Perversion: A metallic or altered taste in the mouth.

• Increased Intraocular Pressure: Can trigger an acute attack of narrow-angle glaucoma.
• Allergic Reactions: Including skin rash, urticaria (hives), or in very rare cases, anaphylaxis.
• Mental Confusion: More common in the elderly, manifesting as disorientation or short-term memory loss.
• Suppression of Lactation: In nursing mothers, it can significantly reduce breast milk production.

> Warning: Stop taking Hyoscyamine and call your doctor immediately if you experience any of these.

1. 1Eye Pain and Redness: This may indicate a sudden increase in eye pressure (acute glaucoma), which can lead to permanent vision loss if not treated.
2. 2Severe Abdominal Pain and Bloating: Could indicate a paralytic ileus (where the intestines stop moving entirely) or toxic megacolon.
3. 3Inability to Urinate: Complete urinary retention is a medical emergency, especially for those with pre-existing bladder outlet obstruction.
4. 4Confusion or Hallucinations: Signs of central anticholinergic toxicity that require immediate evaluation.
5. 5Heatstroke Symptoms: If you stop sweating and develop a high fever, hot/dry skin, and confusion during hot weather, seek emergency care.
6. 6Rapid or Irregular Heartbeat: Palpitations that feel like your heart is racing or skipping beats.

Prolonged use of hyoscyamine may lead to chronic constipation or chronic dry mouth. Chronic dry mouth increases the risk of periodontal disease and oral candidiasis (thrush). There is also emerging clinical research (JAMA Internal Medicine, 2015) suggesting that long-term use of potent anticholinergics may be associated with an increased cumulative risk of developing dementia in older adults. Patients should regularly review the necessity of continued therapy with their healthcare provider.

No FDA black box warnings currently exist for hyoscyamine sulfate. However, it carries significant 'Precautions' and 'Warnings' regarding its use in the elderly and those with specific pre-existing conditions like glaucoma or myasthenia gravis.

Report any unusual symptoms to your healthcare provider.

Hyoscyamine is a potent medication that affects multiple organ systems. It should be used with extreme caution in patients who are sensitive to belladonna alkaloids. Patients should be advised that this medication may cause significant drowsiness or blurred vision, which can impair the ability to perform tasks requiring mental alertness. Furthermore, because hyoscyamine inhibits sweating, patients are at a higher risk for heat exhaustion and heatstroke when exposed to high temperatures or during vigorous exercise.

There are no FDA black box warnings for Hyoscyamine. However, the absence of a black box warning does not imply that the drug is without risk. It is a high-alert medication in certain populations, particularly the elderly and infants.

• Glaucoma Risk: Hyoscyamine can cause pupillary dilation, which may increase intraocular pressure. It is strictly contraindicated in patients with narrow-angle glaucoma.
• Cardiovascular Disease: Use with caution in patients with coronary artery disease, congestive heart failure, or cardiac arrhythmias. The drug-induced increase in heart rate (tachycardia) can worsen these conditions.
• Gastrointestinal Obstruction: In patients with obstructive diseases of the GI tract (such as pyloric stenosis), hyoscyamine can cause complete gastric retention.
• Myasthenia Gravis: Hyoscyamine should be avoided or used with extreme caution in patients with myasthenia gravis, as it can worsen muscle weakness by further blocking acetylcholine receptors at the neuromuscular junction.
• Autonomic Neuropathy: Patients with this condition may experience exaggerated responses to anticholinergic drugs.

While routine blood work is not typically required for hyoscyamine, healthcare providers may perform the following assessments:

• Intraocular Pressure: Periodic eye exams for patients at risk for glaucoma.
• Heart Rate: Monitoring for persistent tachycardia.
• Mental Status: Frequent assessment for confusion or cognitive decline, especially in patients over age 65.
• Bladder Function: Checking for urinary retention in men with prostatic hypertrophy.

Hyoscyamine frequently causes blurred vision and drowsiness. You should not drive, operate heavy machinery, or participate in hazardous activities until you know how this medication affects you. The risk of impairment is increased if hyoscyamine is combined with alcohol or other sedating medications.

Alcohol should be avoided while taking hyoscyamine. Alcohol can potentiate the sedative effects of the drug, leading to extreme drowsiness, dizziness, and an increased risk of falls or accidents.

While hyoscyamine does not typically cause a 'withdrawal' syndrome in the traditional sense, stopping it abruptly after long-term use may result in a 'cholinergic rebound.' This can manifest as increased sweating, nausea, vomiting, and a return of severe gastrointestinal spasms. If you have been taking hyoscyamine regularly, your doctor may recommend a gradual tapering of the dose.

> Important: Discuss all your medical conditions with your healthcare provider before starting Hyoscyamine.

Certain medications should never be combined with hyoscyamine due to the risk of severe, life-threatening interactions:

• Solid Dosage Potassium Chloride: Anticholinergics like hyoscyamine slow down the movement of the gut. If a solid potassium tablet sits in one place in the esophagus or intestine for too long, it can cause severe irritation, ulceration, or perforation. Liquid potassium is generally preferred if a supplement is necessary.

• Other Anticholinergics: Combining hyoscyamine with other drugs that have anticholinergic properties (e.g., benztropine, oxybutynin, or certain antipsychotics) creates an additive effect. This significantly increases the risk of 'anticholinergic toxidrome,' characterized by confusion, severe constipation, and urinary retention.
• Pramlintide: This diabetes medication also slows gastric emptying. Using it with hyoscyamine can lead to severe gastrointestinal motility issues.

• Tricyclic Antidepressants (TCAs): Drugs like amitriptyline have strong anticholinergic side effects. Combining them with hyoscyamine increases the risk of dry mouth, blurred vision, and constipation.
• MAO Inhibitors: Medications like phenelzine may intensify the effects of hyoscyamine by interfering with its metabolism or enhancing its cardiovascular effects.
• Antihistamines: Older antihistamines like diphenhydramine (Benadryl) have significant anticholinergic activity. Concurrent use increases sedation and drying effects.
• Digoxin: Hyoscyamine slows the gut, which may increase the absorption of digoxin tablets, potentially leading to digoxin toxicity. Blood levels of digoxin may need to be monitored.

• Alcohol: As noted, alcohol increases the sedative effects of hyoscyamine.
• High-Fiber Diets: While fiber is often recommended for IBS, taking hyoscyamine may slow the movement of high-fiber bulk through the gut, occasionally leading to increased bloating if the dose is too high.

• Belladonna/Henbane Supplements: Taking herbal supplements containing belladonna alkaloids while on hyoscyamine is dangerous and can lead to overdose.
• St. John's Wort: May theoretically increase the metabolism of hyoscyamine, though clinical data is limited.

Hyoscyamine may interfere with certain diagnostic tests:

• Gastric Secretion Tests: It will suppress acid production, leading to false-negative results in tests designed to measure maximum acid output.
• Radionuclide Gastric Emptying: The drug will significantly delay the results of this test.

For each major interaction, the mechanism usually involves either an additive pharmacodynamic effect (two drugs doing the same thing) or a pharmacokinetic change in GI transit time. Management typically involves dose adjustment or choosing an alternative medication.

> Important: Tell your doctor about ALL medications, supplements, and herbal products you are taking.

Hyoscyamine must NEVER be used in the following conditions:

• Glaucoma (Narrow-Angle): Hyoscyamine causes mydriasis (pupil dilation), which can physically block the drainage angle of the eye, leading to a rapid, painful increase in intraocular pressure and potential blindness.
• Obstructive Uropathy: This includes conditions like bladder neck obstruction due to prostatic hypertrophy. The drug relaxes the bladder muscle (detrusor), making it impossible for the patient to void urine.
• Obstructive GI Disease: Conditions such as achalasia, pyloric stenosis, or paralytic ileus. Slowing the gut further in these patients can lead to rupture or severe toxicity.
• Severe Ulcerative Colitis: In patients with severe UC, hyoscyamine can lead to 'toxic megacolon,' a life-threatening complication where the colon becomes massively dilated.
• Myasthenia Gravis: By blocking acetylcholine at the neuromuscular junction, hyoscyamine can precipitate a myasthenic crisis (severe respiratory and muscle failure).
• Unstable Cardiovascular Status: Especially in acute hemorrhage where the heart rate is already dangerously high.

• Hiatal Hernia with Reflux Esophagitis: Hyoscyamine relaxes the lower esophageal sphincter, which can allow more acid to reflux into the esophagus, worsening heartburn symptoms.
• Autonomic Neuropathy: These patients may have unpredictable responses to the drug.
• Hyperthyroidism: The drug's tendency to increase heart rate can worsen the tachycardia associated with an overactive thyroid.
• Renal or Hepatic Disease: Requires careful monitoring and dose reduction.

Patients who have had a severe allergic reaction to atropine, scopolamine, or other belladonna alkaloids are highly likely to be allergic to hyoscyamine sulfate. Cross-sensitivity is common within this class of medications.

> Important: Your healthcare provider will evaluate your complete medical history before prescribing Hyoscyamine.

Hyoscyamine is classified as FDA Pregnancy Category C. This means that animal reproduction studies have shown an adverse effect on the fetus, or there are no adequate and well-controlled studies in humans. It should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. There is no definitive evidence of teratogenicity (birth defects), but because the drug crosses the placenta, it can affect the fetal heart rate. Use during labor can cause tachycardia in the newborn.

Hyoscyamine is excreted in human breast milk. While the amount the infant receives may be small, anticholinergics can cause side effects in the nursing infant, such as dry mouth or irritability. More importantly, hyoscyamine can suppress prolactin secretion, leading to a significant decrease in milk production. Breastfeeding mothers should discuss alternatives with their healthcare provider.

Hyoscyamine is approved for use in children for certain conditions, but the safety margin is narrow. Infants are particularly sensitive to the toxic effects of belladonna alkaloids. In 2010, the FDA issued warnings regarding the use of such products for infant colic due to reports of serious adverse events, including seizures and difficulty breathing. Pediatric dosing must be calculated precisely by a doctor.

Elderly patients (65+) are at the highest risk for adverse effects. The central nervous system effects (confusion, delirium) and the peripheral effects (urinary retention, constipation) are significantly more pronounced in this population. Hyoscyamine is listed on the Beers Criteria as a 'potentially inappropriate medication' for the elderly. If it must be used, clinicians are advised to 'start low and go slow.'

Since hyoscyamine is largely eliminated unchanged in the urine, patients with a GFR below 60 mL/min are at risk for drug accumulation. This can lead to prolonged side effects and a higher risk of toxicity. Dose reductions of 25-50% may be necessary depending on the degree of impairment.

While not the primary route of elimination, severe hepatic impairment (Child-Pugh Class C) can alter the metabolism of belladonna alkaloids. Patients with liver cirrhosis should be monitored closely for signs of increased sedation or cognitive impairment.

> Important: Special populations require individualized medical assessment.

Hyoscyamine is a competitive inhibitor of the muscarinic acetylcholine receptors. It specifically targets the M1 (gastric glands, CNS), M2 (heart), and M3 (smooth muscle, exocrine glands) receptors. By competing with acetylcholine for these binding sites, it prevents the activation of the G-protein coupled receptor cascade. In the gut, this prevents the increase in intracellular calcium that normally leads to smooth muscle contraction, thereby acting as an antispasmodic. In the salivary glands, it prevents the activation of secretory pathways, leading to dryness.

The onset of action for oral immediate-release hyoscyamine is approximately 20 to 30 minutes. The duration of effect typically lasts 4 to 6 hours. For extended-release formulations, the effect is sustained over 12 hours. There is a clear dose-response relationship; higher doses lead to more significant heart rate increases and more profound inhibition of secretions. Tolerance to the antispasmodic effects is rare, but patients may become somewhat accustomed to the side effects over time.
