Tobcharm Antifungal Treatment Cream

For most fungal infections of the skin, the standard application protocol for undecylenic acid involves a consistent twice-daily regimen.

• Tinea Pedis (Athlete's Foot): Apply a thin layer of the cream, ointment, or solution to the affected area and the skin immediately surrounding it twice daily (morning and night). Treatment should typically continue for 4 weeks.
• Tinea Cruris (Jock Itch): Apply twice daily for a period of 2 weeks. It is vital to continue application even if symptoms disappear earlier to ensure the fungus is fully eradicated.
• Tinea Corporis (Ringworm): Apply twice daily for 4 weeks.

Undecylenic acid is generally considered safe for children aged 2 years and older when used under adult supervision. The dosage and frequency are identical to the adult dosage (twice daily).

• Children under 2 years: Safety and efficacy have not been established for infants. Consult a pediatrician before use, as infant skin is thinner and more prone to systemic absorption of topical agents.

Renal Impairment

Since systemic absorption is minimal, no dosage adjustments are required for patients with kidney disease. However, patients should avoid applying the product to large open wounds where absorption could theoretically occur.

Hepatic Impairment

No dosage adjustments are necessary for patients with liver disease due to the localized nature of the treatment.

Elderly Patients

No specific adjustments are needed for geriatric patients. However, healthcare providers should monitor for skin thinning or fragility, which may increase the risk of local irritation.

Proper application technique is critical for the success of antifungal therapy:

1. 1Clean and Dry: Wash the affected area with mild soap and water. Pat it completely dry before application. Fungi thrive in moist environments, so drying is a therapeutic step in itself.
2. 2Application: Apply a thin layer. Using more than directed will not speed up the healing process but may increase the risk of skin irritation.
3. 3Hygiene: For athlete's foot, pay special attention to the spaces between the toes. Wear well-fitting, ventilated shoes and change socks at least once daily.
4. 4Hands: Wash your hands immediately after applying the medication to prevent spreading the infection to other parts of your body or to other people.
5. 5Storage: Store the product at room temperature (15°C to 30°C / 59°F to 86°F). Keep the container tightly closed and away from excessive heat or direct sunlight.

If you miss a dose, apply it as soon as you remember. If it is almost time for your next scheduled application, skip the missed dose and resume your regular schedule. Do not 'double up' the amount applied to make up for a missed dose.

Topical overdose is unlikely. However, if the medication is accidentally ingested, it may cause gastrointestinal distress, nausea, or vomiting.

• Emergency Measures: In case of accidental ingestion, contact a Poison Control Center or seek emergency medical attention immediately. If the product gets into the eyes, flush with copious amounts of cool water for at least 15 minutes.

> Important: Follow your healthcare provider's dosing instructions. Do not adjust your dose or stop treatment early without medical guidance, as this increases the risk of the infection returning.

Undecylenic acid is generally well-tolerated, but local skin reactions are the most frequently reported side effects. These typically occur at the site of application and include:

• Mild Burning or Stinging: This sensation often occurs immediately after application and usually subsides within a few minutes. It is more common if the skin is cracked or raw.
• Skin Redness (Erythema): A mild pinkish or reddish tint to the treated skin may develop as the area responds to the medication.
• Dryness or Flaking: As the fungal infection resolves, the skin may appear dry or begin to peel. This is often a sign of the old, infected skin shedding.

• Pruritus (Itching): While the medication is intended to stop the itch of the fungus, some patients may experience a different type of itching related to contact with the medication itself.
• Contact Dermatitis: A localized allergic-like reaction where the skin becomes itchy, red, and potentially develops small bumps or blisters.
• Tenderness: The treated area may feel slightly sore or sensitive to the touch.

• Severe Skin Irritation: Intense burning, pain, or swelling that does not resolve quickly.
• Hives (Urticaria): Raised, itchy welts that may appear on the application site or elsewhere on the body, indicating a systemic hypersensitivity.
• Oozing or Blistering: The development of fluid-filled sacs at the application site.

While extremely rare for a topical antifungal, serious reactions can occur.

> Warning: Stop taking Undecylenic Acid and call your doctor immediately if you experience any of these:

• Anaphylaxis: Signs include difficulty breathing, swelling of the face, lips, tongue, or throat, and severe dizziness. This is a life-threatening emergency.
• Severe Skin Sloughing: If large areas of skin begin to peel off or look like a burn, it may indicate a severe dermatological reaction.
• Spreading Infection: If the redness or swelling begins to spread rapidly away from the original site, it may indicate a secondary bacterial infection (cellulitis).
• Fever and Chills: Systemic symptoms accompanying a skin rash may indicate a more serious underlying condition.

Undecylenic acid is not intended for long-term or chronic use. Prolonged use (beyond the recommended 4 weeks) may lead to:

• Skin Sensitization: The skin may become increasingly sensitive to the ingredient, making future use impossible due to immediate allergic reactions.
• Fungal Resistance: Although less common than with antibiotics, overuse of antifungals can theoretically lead to the selection of resistant fungal strains.
• Skin Thinning: While not a steroid, chronic irritation of the skin can disrupt the natural barrier function over time.

No FDA black box warnings have been issued for undecylenic acid. It is considered to have a high safety profile when used as directed on the skin.

Report any unusual symptoms or worsening of your condition to your healthcare provider. You may also report side effects to the FDA at 1-800-FDA-1088.

Undecylenic acid is for external use only. It must never be taken by mouth, used in the eyes, or applied to mucous membranes (such as inside the nose, mouth, or vaginal area). If the product comes into contact with these areas, rinse thoroughly with water.

Patients with certain underlying health conditions must exercise extreme caution. Specifically, individuals with diabetes mellitus or peripheral vascular disease (poor blood circulation) should not use this product unless specifically directed by a healthcare provider. In these populations, minor skin irritations can lead to serious complications, such as non-healing ulcers or secondary bacterial infections.

There are no FDA black box warnings for Undecylenic Acid. It is widely available as an over-the-counter medication due to its established safety record over several decades of use.

• Allergic Reactions: If you have a known allergy to castor oil or other fatty acids, you may be at a higher risk for hypersensitivity. Discontinue use if a rash or irritation develops.
• Broken or Irritated Skin: Do not apply undecylenic acid to skin that is severely broken, oozing, or shows signs of a deep bacterial infection (such as pus or warmth). The medication is intended for superficial fungal infections only.
• Asthma and Sulfite Sensitivity: Some formulations of undecylenic acid may contain sulfites as preservatives, which can trigger asthma-like symptoms in sensitive individuals.
• Deep Wounds: This medication is not suitable for treating fungal infections of the scalp or nails unless directed by a physician, as these often require systemic (oral) therapy to reach the source of the infection.

For standard OTC use, laboratory monitoring (like blood counts or liver tests) is not required. However, clinical monitoring is essential:

• Efficacy Monitoring: If there is no improvement in athlete's foot or ringworm after 4 weeks, or jock itch after 2 weeks, the treatment should be stopped and a doctor consulted.
• Skin Integrity: Regularly inspect the treated area for signs of excessive irritation or secondary infection.

Undecylenic acid is a topical agent and does not cross the blood-brain barrier in significant amounts. It has no known effect on the ability to drive or operate heavy machinery.

There are no known direct interactions between topical undecylenic acid and alcohol consumption. However, alcohol can sometimes exacerbate skin flushing or dehydration, which might indirectly affect skin comfort.

There is no need to taper undecylenic acid. However, it is vital to complete the full course of treatment (usually 2-4 weeks) even if symptoms resolve within the first few days. Stopping too early can allow remaining fungal spores to regrow, leading to a recurrence of the infection.

> Important: Discuss all your medical conditions, especially diabetes or circulation problems, with your healthcare provider before starting Undecylenic Acid.

There are no known systemic drug-drug interactions that are strictly contraindicated with topical undecylenic acid. However, it should not be used simultaneously on the same area of skin with other topical medications unless directed by a doctor, as this can lead to chemical incompatibilities or severe skin irritation.

• Topical Corticosteroids: Using high-potency topical steroids (like clobetasol) along with undecylenic acid can mask the signs of a worsening fungal infection. Steroids can suppress the immune response in the skin, allowing the fungus to spread while the inflammation appears reduced. If both are needed, they should usually be applied at different times of the day.
• Other Antifungals: Combining different topical antifungals (e.g., mixing undecylenic acid with terbinafine) has not been studied for safety or efficacy and may increase the risk of contact dermatitis.

• Skin Cleansers and Medicated Soaps: Using harsh or abrasive soaps, or those containing salicylic acid or benzoyl peroxide, in the same area as undecylenic acid may cause excessive drying and irritation of the skin barrier.
• Occlusive Dressings: Covering the treated area with airtight bandages or plastic wraps can significantly increase the absorption of the acid and the risk of local irritation. Unless directed by a doctor, the treated area should be left open to the air or covered only with loose cotton clothing.

There are no known interactions between topical undecylenic acid and specific foods, including grapefruit, dairy, or caffeine. Because the drug is not absorbed systemically, dietary factors do not influence its metabolism.

• Tea Tree Oil: Some patients use tea tree oil as a natural antifungal. Using it concurrently with undecylenic acid may increase the risk of skin sensitization or allergic contact dermatitis.
• St. John's Wort: No known interaction, as there is no systemic CYP450 involvement.

Undecylenic acid is not known to interfere with standard blood or urine laboratory tests. It does not affect glucose monitoring in diabetic patients, though the physical presence of the ointment on the fingers could theoretically contaminate a finger-stick test if the hands are not washed.

Management Strategy

To minimize the risk of interactions:

1. 1Clear the Surface: Ensure the skin is free of other lotions, sunscreens, or cosmetics before applying undecylenic acid.
2. 2Sequential Application: If you must use another topical product, wait at least 30 to 60 minutes between applications.
3. 3Communication: Always inform your pharmacist if you are using multiple topical treatments for the same condition.

> Important: Tell your doctor about ALL medications, supplements, and herbal products you are taking, even those applied to the skin.

Undecylenic acid must NEVER be used in the following circumstances:

• Known Hypersensitivity: If you have had a previous allergic reaction to undecylenic acid, zinc undecylenate, or any inactive ingredient in the formulation (such as lanolin, parabens, or fragrance), you must not use this product. Anaphylaxis or severe contact dermatitis could result.
• Ophthalmic Use: This medication is strictly prohibited for use in or near the eyes. The acid can cause severe corneal irritation or chemical burns.
• Mucous Membranes: It should not be applied to the vaginal, anal, or oral mucosa, as it is too irritating for these sensitive tissues.

Conditions requiring a careful risk-benefit analysis by a physician include:

• Diabetes Mellitus: Patients with diabetes often have reduced sensation in their feet (neuropathy). They may not feel the irritation caused by the medication, which could lead to a 'silent' chemical burn or ulcer.
• Peripheral Vascular Disease (PVD): Poor circulation slows the healing process. Any irritation caused by the antifungal could lead to a chronic wound or secondary infection.
• Extensive Skin Breakdown: If the skin is severely denuded or exhibits deep fissures, the risk of systemic absorption and intense local pain increases.

There is a potential for cross-sensitivity among various fatty acid derivatives. Patients who react poorly to sebacic acid or other medium-chain fatty acids in cosmetics should use undecylenic acid with caution. Additionally, many undecylenic acid ointments contain lanolin (wool fat), which is a common allergen. Patients with a known wool allergy should check the inactive ingredients list carefully.

> Important: Your healthcare provider will evaluate your complete medical history, including your circulatory health, before prescribing or recommending Undecylenic Acid.

FDA Pregnancy Category: Not Formally Assigned (typically viewed as Category B or C depending on the specific product).

There are no adequate and well-controlled studies of topical undecylenic acid in pregnant women. However, because systemic absorption is negligible when applied to intact skin, the risk to the developing fetus is considered extremely low. According to the principles of clinical pharmacology, topical fatty acids are unlikely to reach the systemic circulation in concentrations that would cross the placenta. Nevertheless, it should be used during pregnancy only if clearly needed and after consultation with a healthcare provider.

It is not known whether topical undecylenic acid is excreted in human milk. However, given the minimal systemic absorption, it is unlikely to pose a risk to the nursing infant.

• Precaution: If applying the medication to the chest or breast area, ensure the medication is thoroughly washed off before breastfeeding to prevent the infant from accidentally ingesting the drug or coming into direct skin contact with it.

Undecylenic acid is approved for use in children aged 2 to 12 years for the treatment of ringworm and athlete's foot.

• Safety: It has a long history of safe use in children.
• Supervision: Children should be supervised during application to ensure they do not rub the medication into their eyes or mouth.
• Infants: Use in children under 2 years of age is not recommended without a direct order from a pediatrician.

Elderly patients may use undecylenic acid safely, but they should be monitored for:

• Skin Fragility: Older skin is thinner and may be more easily irritated by the acidic nature of the medication.
• Circulation Issues: As mentioned, the prevalence of PVD and diabetes is higher in this population, necessitating caution.

No dosage adjustment is required. The amount of drug that reaches the kidneys is clinically insignificant in patients with normal skin barrier function.

No dosage adjustment is required. The liver is not significantly involved in the clearance of this topical medication.

> Important: Special populations, particularly those with metabolic or circulatory disorders, require individualized medical assessment before using any antifungal therapy.

Undecylenic acid is a medium-chain fatty acid that exerts fungistatic activity. Its primary molecular mechanism involves the disruption of the fungal cell membrane. The 11-carbon chain of the acid allows it to partition into the lipid bilayer of the fungal plasma membrane. This insertion causes physical stress and changes in the fluidity of the membrane, leading to the formation of 'pores' or disorganized regions.

Additionally, undecylenic acid inhibits the enzyme fatty acid synthase in certain fungal species, preventing the fungus from producing its own essential lipids. It also interferes with the pH gradient across the cell membrane, which is vital for nutrient transport and energy production (ATP synthesis). In Candida species, it specifically inhibits the yeast-to-hyphae transition by interfering with the signaling pathways required for filamentation.

The dose-response relationship of undecylenic acid is localized. Higher concentrations (up to 25%) provide more robust fungistatic activity. The time to onset of symptomatic relief (reduction in itching) is typically 2 to 7 days, though the eradication of the fungus takes much longer. Tolerance to undecylenic acid has not been widely reported in clinical literature, making it a reliable option for recurrent infections.

• Molecular Formula: C11H20O2
• Molecular Weight: 184.28 g/mol
• Solubility: Practically insoluble in water; very soluble in alcohol, chloroform, and ether.
• Structure: It is a 10-carbon unsaturated chain with a terminal carboxylic acid group. The unsaturation (double bond) at the 10th position is critical for its antifungal potency compared to saturated fatty acids like lauric acid.

Undecylenic acid is classified as a Topical Antifungal Fatty Acid. It is often grouped with other OTC antifungals like tolnaftate and the azoles, though its chemical structure is distinct. It is frequently formulated with Zinc Undecylenate to enhance its therapeutic effect through the astringent properties of zinc.
